Traction — applying a controlled pull along the long axis of a limb — is the standard access for hip fracture fixation, hip arthroplasty, and tibial shaft fixation. The position is held by a traction table with a perineal post as the counter-traction anchor; mechanically elegant and neurally dangerous.

The perineal post
The perineal post compresses the pudendal nerve, the scrotum or labia, and — with excessive hip flexion — the sciatic nerve. A gel orthopedic perineal column pad distributes the load across a larger surface area and is now the standard of care in published position-injury reduction programs.
The foot holder and frame families
The foot is held in a boot or ankle strap; if too tight it compresses the anterior ankle, if too loose the foot slides out mid-case. Traction frames come in standard, carbon-fiber radiolucent, and hydraulic/mechanical-adjustment variants. Most traction patients are elderly — pad more, re-check more often, and assume the case will run longer than booked.
